Program of Study Form

The GS-6 Form (Program of Study Form) provides a record of the program of study the graduate student will complete to earn the degree.  The GS-6 Form is due before the end of the second term of enrollment after being admitted as a graduate student (including summer) or after completing 12 semester credits, whichever comes first. This applies to part-time and full-time students. All necessary signatures must be obtained prior to the form being submitted to the Graduate School.  Contact your advisor at MAccAdvisor@business.colostate.edu or 970-491-4612 if you have questions about this form.

You must complete the GS-6 form electronically by using RAMweb. Instructions can be found on the Graduate School's web site (.pdf). Ask the MAcc advisor for assistance prior to completing your GS6 as it is a document that must be submitted to Graduate School error free.

INSTRUCTIONS FOR GS6

  1. The form can be found on the Graduate School website.
  2. Fill in name, ID, phone, email, and semester admitted. Program is ACCY-MACC, degree is Master of Accountancy. Leave the area of study line blank.
  3. The courses to be listed are only those ten courses you and your advisor have agreed will constitute your program of study. They should be listed under either the “prior” or “after” admission part as appropriate. The total credits should add to 30. Do not list prerequisites.
  4. Our program does not require a faculty committee, or have non-course requirements, so ignore those parts. However, the name of the advisor must be filled in.
  5. Bring the completed form to the MAcc Advisor prior to submitting the form for any further signatures.
